1. REQUIRED DOCUMENTS FOR APPLICATION AMENDMENTS INCLUDES:

- Power of attorney attached here

- Application for collective mark registration attached here

- Model of collective mark ( photos file jpg, pdf, psd,...)

- Regulation for use collective mark, includes required contents:

•  Information about mark owner
• Criteria for becoming a member of a collective organization
• List of organizations and individuals permitted to use the mark;
• Conditions for using the mark;
• Measures to handle violations of regulations on trademark use.

- A description of the characteristic (or distinctive) nature and quality of the product bearing the mark.

- Document of the People's Committee of the province or centrally city allowing the registration of the mark (Requirement for a registered mark being a certification mark containing a place name or other sign indicating the geographical origin of product use registered mark).

- Map identifying the territory (if the registered mark is a mark certifying the geographical origin of the product).

- Photos of Certificate of business/cooperative registration
